There are many events in Islamic history that could have led to signifigant word meaning changes, and a low level of education can cause pockets of misunderstanding that can be agrivated by a frequent need for translation.

So some renditions of this so called hadith might indicate such a situation but only if the word frequently translated as "ass" was actually "pisshole" and he asked "did you put your penis in your wifes pisshole" and then the mans response "no way thats impossible" and the prophets response "its not impossible but its harmful so guard against it" makes sense. Otherwize there is a chance he condemned the way his own mother concieved and gave birth to him, as standardized female genitalia is a western phenominon that only dates back to the 1930's.
But if he said penis in the piss hole he didnt forbid women with contenance problems from using urethral plugs or various forms of play using other smaller objects.
Another time he got angry and spoke loudly (if he actually spoke loudly in the first instance) was to scold people helping new reverts wounded in battle to understand their obligations concerning wudu in which they had killed a man with an open brain injury in which they had poured water directly onto his brain. He told them they had murdered the man and that he could have done tayamum.
